This web page is dedicated to assisting the District churches with the filing of required information in addition to providing forms which can be utilized for administrative purposes by your church staff members. Additional forms will be added as they are identified. Watch your monthly District Newsletter for information about new forms being added to the site.

Certificate of Lay Delegate	All churches that are an integral part of The Christian and Missionary Alliance may send the pastor and two lay delegates to General Council. If church voting membership is 150 or more, another lay delegate may be sent for each additional 100 members. Please submit this form to the District Office electronically for each lay delegate from your church.	Lay Delegate
Annual Missions Conference Reports	District churches are asked to gather and report information at the conclusion of their annual Missions Conference. The "Mission Conference Report" should be completed and sent to the District Office. National Office is requested that each church complete a report following Missions Conference. Official Workers Scholarship Application	A Scholarship Fund has been establish by DEXCOM to assist Official Workers in the District with leadership development. Qualifications and the approval process are document as part of the WGL District Policy Manual.	OW Scholarship Application Form
